WHAT TO BRING
Walk in prepared, walk out informed.
In order to make our first meeting as productive as possible, we ask new patients to come prepared.
We want you to feel comfortable and confident when you first visit the Care Life Clinic, with a complete understanding of what to expect. Please arrive 15 minutes before your appointment time in order to complete your checkin process and completed new patient forms. We encourage you to print ‘New Patient Forms’ from our website carelifeclinic.com, fill these forms at home and bring to your first visit to save time.
For an evaluation of Vein condition please bring (or wear) loose, comfortable shorts (for doctor to be able to examine your legs properly).
EVALUATION & DIAGNOSIS Bring on the day of first visit
- A list of your current medications
- Information about what treatments and medications have been tried in the past
- Medical records from prior visits that you think may be helpful
Before Your Visit
- Drink plenty of water throughout the day. This will help us get a better ultrasound image as we can see the veins more clearly when the body is well hydrated.
- Please eat a light meal before your visit.
Prepare For the Day of Vein Treatment
Returning to a healthy life will be much easier when you are prepared for your vein treatment. Follow these instructions below to allow for maximum effectiveness of your treatment.

- Bring compression stockings (if you have any) to every visit.
- Do not apply lotion/moisturizer on your day of treatment.
- Hydrate yourself throughout the day. Drink water prior to your visit.
- Please make sure to eat something.
When you come in to our clinics for your vein treatment procedure, we ask that you bring your compression stockings to wear once the procedure is complete. If you do not have compression stockings, you can talk with one of our representatives in the clinic about getting fitted for stockings to purchase.
We recommend that you wear loose and comfortable clothes to your appointment. You may be asked to bring in a shorts to wear during the treatment. This will ensure you are comfortable throughout your treatment and will allow the specialists to access the affected area easier. Although we do not use general anesthesia for any of our treatments, we encourage you to have someone escort you to your appointment so they can drive you home once it is complete.

At the top of your treatment team is a vein specialist whose primary concern is making sure you feel at ease throughout the entire process, from initial consultation to ongoing treatments and follow-up appointments.
For Vein health appointment at your first meeting, your United States Board certified and fellowship trained and experienced doctor will discuss your health history, examine your legs, evaluate your condition, and answer all of your questions. You’ll discuss whether or not an ultrasound and/or mapping are required at a follow-up appointment, which will assist in a diagnosis, treatment plan, and estimated number of treatments.
For Mental health appointment you will meet your American Board Certified most experienced psychiatrist (over 30 years experience), who ranked as a ‘top doctor’ in United States in his field by his peers. Dr. Naseem Chaudhry is considered to be one of the world’s best and most sought after medication management (psychopharmacologist) psychiatrist. He regularly provides Telemedicine consults to patients from all over the world, in addition to in-person consultations in Lahore. First appointment is longer in order to thoroughly go over your history and background. He provides an in-depth analysis and understanding of your core issues before starting any medications.
VARICOSE VEINS and OTHER CONDITIONS TREATMENT COST
You’ll meet with an office manager to talk through your costs of treatment and how it applies to your specific diagnosis and treatment plan. They’ll also make sure you know when your treatment will begin, and what to expect at that time. A psychiatric consultation is Rs. 3,000.
